A GDC Specialist Orthodontist vs a standard Invisalign provider: what’s the real difference?

BOOK NOW

A standard Invisalign provider is any dentist who has done Align Technology’s product training. A GDC Specialist Orthodontist has completed years of further university training and is on the General Dental Council’s specialist list. At The London Smile Clinic in Fitzrovia, your aligners are planned by Dr Pratik Sharma, who holds that registration.

Here’s a fact that surprises most people. Almost any dentist in the UK can offer Invisalign. The clear aligners are a product, and the company runs courses that let general dentists provide them after relatively short training.

That’s not a criticism. Plenty of straightforward cases go beautifully in those hands. But it does mean the words “Invisalign provider” tell you about the brand of aligner, not about the depth of training of the person planning your teeth.

So what’s the actual difference, and when does it matter? Stay with me.

What “Registered Specialist” really means

Any dentist can move teeth. Only a clinician who has completed several years of additional, formal orthodontic training and is entered on the GDC’s specialist list may call themselves a Registered Specialist in Orthodontics.

It’s a protected title, backed by qualification and assessment, not a marketing label. For you, it’s a quick way to know that the person planning your case has trained specifically, and at length, to move teeth, predict how they’ll respond and handle it when they don’t behave.

Dr Pratik Sharma holds that registration. He qualified with honours from Guy’s Dental Hospital in 1999 and took his MSc in Orthodontics, with distinction, at the Royal London Hospital.

The person planning your teeth, not just the product

Invisalign is a tool. The result depends almost entirely on the plan behind it: how each tooth is staged to move, where attachments go, how the bite is managed, what the finish should look like.

A specialist plans that himself, case by case, drawing on years of treating teeth that didn’t read from the textbook. Dr Sharma divides his time between The London Smile Clinic and the Royal London Hospital, where he’s a Consultant and Senior Clinical Lecturer training the next generation of orthodontic specialists and consultants.

Read that again. He trains the consultants. His research has been published in leading journals and recognised with awards from the American Journal of Orthodontics and the British Orthodontic Society.

Where the difference shows up

For a mild case, both routes can work well. The gap opens up when the case isn’t simple, and you often can’t tell in advance which yours is.

What you’re comparing Standard Invisalign provider GDC Specialist Orthodontist
Training Product training from Align Technology Years of university orthodontic training, on the GDC specialist list
Best suited to Mild to moderate cases The full range, including complex bites and relapses
Treatment options Usually clear aligners only Aligners, lingual braces, tooth-coloured fixed braces
Bite and jaw problems Often referred elsewhere Planned in-house
If a case goes off-plan May reach the limit of training Handled within the specialist’s remit

That last row is the one that matters most. A specialist doesn’t just start a case well. He knows what to do when a tooth refuses to move, when the bite needs more thought, or when a previous treatment has relapsed.

More than one way to straighten teeth

A general provider often offers aligners and little else. A specialist chooses the right appliance for the case.

Dr Sharma particularly enjoys discreet treatments. As well as Invisalign, he provides lingual braces, fixed behind the teeth where nobody can see them, through the Incognito system, and tooth-coloured fixed braces for cases where fixed appliances work better than aligners.

Lingual braces are the best-kept secret in orthodontics. They straighten teeth as effectively as conventional braces while staying completely hidden, which suits professionals who can’t have visible metalwork. They’re also technically demanding to fit and adjust, which is exactly why specialist-level skill earns its place.

Does the difference cost more?

Not necessarily, and this is where assumptions trip people up.

Orthodontic fees in the UK vary widely with the complexity of the case, commonly from around £2,500 for a straightforward course to £6,000 or more for complex or specialist treatment. A simple case planned by a specialist isn’t automatically dearer than the same case elsewhere. What you’re really buying is the certainty that whoever you’ve chosen can handle yours, whatever it turns out to be.

The London Smile Clinic confirms your exact fee after a consultation. You can see the wider picture on our orthodontics pages first.

Which one do you actually need?

Be honest about your case, and accept that you may not know how complex it is until it’s assessed.

If your teeth need only minor alignment, an experienced general provider may serve you perfectly well. If your case has been called difficult, involves your bite or jaw, or is a relapse after braces years ago, a specialist opinion is worth seeking before you commit. And if you simply want the reassurance that the person planning your smile trains other orthodontists, that decision makes itself.

As of June 2026, Dr Pratik Sharma sees orthodontic patients at The London Smile Clinic in Fitzrovia, a short distance from Marylebone, Bloomsbury and Regent Street. The simplest next step is a consultation, where he can tell you which level of care your teeth genuinely need.

FAQ

Is Invisalign from a specialist orthodontist better than from a general dentist?

For mild cases, both can work well. For anything more involved, a specialist has a clear edge, because the result depends on the treatment plan rather than the aligner brand. Dr Pratik Sharma is a GDC Registered Specialist in Orthodontics and a Consultant at the Royal London Hospital, so complex bites, relapses and difficult movements are planned by someone trained specifically for them.

What can a specialist orthodontist treat that a standard provider can’t?

A specialist handles the full range, including awkward bites, jaw-related problems and cases that have relapsed after previous braces. They can also offer lingual braces and tooth-coloured fixed braces, not just aligners, and choose the appliance that suits your case best. At The London Smile Clinic, Dr Sharma plans these in-house rather than referring them elsewhere.

Are lingual braces as effective as normal braces?

Yes. Lingual braces, such as the Incognito system, are fixed behind your teeth and straighten them as effectively as conventional braces, with the advantage of being completely hidden from view. They’re technically demanding to fit and adjust, which is why specialist-level experience matters. Dr Pratik Sharma provides lingual braces at The London Smile Clinic in Fitzrovia.

How much does specialist orthodontic treatment cost in London?

UK orthodontic fees range widely, commonly from around £2,500 for a straightforward course to £6,000 or more for complex or specialist treatment. A simple case planned by a specialist isn’t automatically more expensive than elsewhere. The London Smile Clinic confirms your exact fee after a consultation, where Dr Sharma sets out the plan and options before you decide anything.

How do I know if I need a specialist or a general dentist for Invisalign?

If your alignment needs are minor, an experienced general provider can often help. If your case is complex, has been called difficult, or involves your bite or jaw, a Registered Specialist opinion is worth seeking. The clearest step is a consultation, where Dr Sharma can assess your teeth and tell you honestly which level of care suits your situation.

I had my smile makeover in 2006, and its now 2024, and I am still absolutely thrilled with the work that Dr Tim Bradstock Smith did for me. My gums were in a mess, always bleeding, I had trigeminal nerve pain on both sides of my jaw. He transformed my life with his work, & my confidence, he took all the problems away. I never stopped smiling. Love my teeth now. I have recently been back for a hygenist visit is the best one I've ever had. I took my husband this time for a consultation, I figured mine have lasted 18 years and are showing no decline, so I'd rather him go to Tim too. They have an offer on a free consultation, and £100 off your own bill, which they honoured with a smile. Lovely friendly reception staff. I highly recommend speaking to them, special thanks to Tim & Dawn & the team & our lovely hygenists. x

I knocked out my perfect bridge six days before Christmas and they went out of their way to sort it out in one day, working overtime. Not only did Tim save the day, but they saved my Christmas! I’d already been asked many times who made my bridge 10 years ago - not that you’d have noticed, it was perfect. I don’t know why anyone would go anywhere else.
